Are you presenting with a fever, new onset of cough, worsening of chronic cough, shortness of breath, difficulty breathing.
Have you had close contact with anyone with acute respiratory illness or travelled outside of Canada in the past 14 days.
Do you have a confirmed case of Covid -19 or had close contact with a confirmed case of Covid-19.
Do you have two (2) or more of the following: running nose/sneezing
nasal congestions
hoarse voice
difficulty swallowing
decrease or loss of sense of smell
chill
headaches
unexplained fatigue/ malaise
diarrhea
abdominal pain
nausea/ vomiting
Are you over 65 year of age?
if so are you experiencing delirium, falls, acute functional decline or worsening of chronic condition?
If you have answered YES to any one these questions please be advised to :
Not attend the office in person for the next 14 days.
Complete the ONT gov’t self assessment.
Contact appropriate authorities such as physicians or local health offices.
